Registration for the 2020-2021 school year begins on Monday, June 29th.

Register for FLASH There are 2 ways to register for the program: 1) Registration paperwork can be picked up from The Community Pavilion, The Racquet, Fitness & Gymnastics Center, as well as The Oakview Center.  Once paperwork is completed it can be submitted to any of these facilities. 2) Registration paperwork can be printed from the links below.  Please print the paperwork and fill out all appropriate information before handing paperwork into any of the above park district facilities.

Registration Fee There is an annual $55 non-refundable registration fee required at the time of registration. 

How Soon Can We Start? FLASH requires at least 48 hours notice from the time that your completed paperwork is returned before your child(ren) can start the program.  Parents can NOT turn in paperwork one day and have their child start the next.  We need time to process the paperwork, alert the school and staff that your child will be attending, as well as make sure we have an appropriate number of staff present. All parents (who list an email address) will receive a confirmation email within in 48 hours of receiving your completed registration packet.

Parent Handbook All parents are required to read through the parent handbook before registering for FLASH.  The handbook goes through all policies and procedures in of the program in detail and will serve as a good reference for any questions you may have regarding these practices.  Thorough understanding of the parent handbook will also help you to avoid fines for violating our policies. Will request the name and contact information for all persons you would like to be contacted in case of emergency (in the event that we cannot contact the parents), as well as persons allowed to pick your child up from the program.

Enrollment Agreement* 
Requests your child's set schedule at the program, as well as ensures that you have read and understand all of the policies in the parents handbook.  We will also use this form to keep a record of late pick ups from the program.

Medication Authorization
This form must be completed if your child is required to take any form of medication during the program, including an inhaler or epi-pen.

Direct Debit Credit/Debit 

Please fill out this form if you would like to enroll in our direct debit payment agreement.  Your tuition payments will be automatically debited from your credit or debit card on the 1st of the month.
